This privacy policy explains how Price Street Medical collects, stores, uses and shares your personal and health information in accordance with the Privacy Act 1988 and the Australian Privacy Principles (APPs).

In accordance with the privacy act 1988, all health records collected at this practice are treated as confidential.It is the policy of this practice to maintain security of personal health information at all times and to ensure that the information is only available to authorised members of staff. 

To protect your privacy, we are unable to give out the details of investigation and results over the phone or at the front counter.  If you have questions about how we handle your personal health information or need to arrange access to your records, please ask the staff or your doctor, as appropriate.

Why and When Your Consent is Necessary

When you become a patient at our clinic, you provide consent for our doctors and staff to collect and use your personal information for the purpose of providing medical care. We will seek additional consent if we need to use your information for anything beyond standard care.

• Through your My Health Record, Shared Health Summary, or eTP

How We Store Your Information

Your medical records may be stored as:

• Paper files (which are securely destroyed when no longer required)
  
• Visual and audio files (e.g. scans, photos) 

All digital information is stored using password-protected software, with individual login credentials for each practitioner and staff member.

Accessing or Correcting Your Information

You can request access to or correction of your medical records.

• Requests must be made in writing or verbally to our reception
  
• We aim to respond within 30 days
  
• A fee may apply for the release of information
  
• You can update your contact details or health history at any time by informing our staff 

Dealing with Us Anonymously

You have the right to remain anonymous or use a pseudonym unless it is impractical or we are legally required to identify you.

Data Security

We take all reasonable steps to secure your personal information and prevent unauthorised access, modification or disclosure.
